I understand that, if employed, any misrepresentation, false statement or omission later discovered will be grounds for termination. I authorize A.G. Williams Painting Co, Inc. to investigate my employment record and background, including criminal conviction records, and I hereby release and hold A.G. Williams Painting Co, Inc. and all parties who provide such records or information harmless from all liabilities arising from such disclosures. I understand that, if I receive a conditional offer of employment, I will be required to undergo a drug screen, and I agree to take such a test. I also understand that a positive result or a finding of an adulterated or substituted specimen on the post-offer drug test shall be grounds for rescinding any conditional offer of employment. A refusal to take the test shall be considered a positive result. Further, I am aware that during the course of my employment I may be subjected to drug and alcohol testing based on the reasonable suspicion of the Company. I understand that if I am hired, A.G. Williams Painting Co, Inc. may require me to undergo alcohol testing within the guidelines of its personnel practices, participation in any New York Sate of Workers’ Compensation drug-free workplace program, or to the extent permitted by law, and I agree to cooperate in such testing. I understand that refusal to do so may result in immediate termination and a positive result or a finding of an adulterated or substituted specimen, will result in disciplinary action up to and including termination from employment. I understand that, if hired, my employment is at will, which means that either I or the company may terminate the employment relationship at any time, for any or no reason, and with or without notice.
